POSITION:                                   Healthcare Facilitator

 

LOCATION:                                  District Wide – As Assigned

 

REPORTS TO:                               Principal or Designee

 

JOB GOAL:

  • To assist the Principal in providing the fullest possible educational opportunity for each student by minimizing absence due to illness and creating a climate of health and well-being in the district schools.      

 

QUALIFICATIONS:

Qualifications to perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required to be successful:

  • Valid Arizona CNA license required
  • IVP Fingerprint Clearance card is required
  • Two years of nursing experience required
  • Current CPR certification
  • Must be able to move about campus in case of emergencies; may have to do some lifting and transporting of students
  • Must have the ability to work independently and make sound judgments and decisions in a wide variety of situations under pressure

RESPONSIBILITIES:

Responsibilities and duties may include, but are not limited to, the following:

  • Assumes the responsibility for appropriate intervention, management and/or referral of all students
  • Provides relevant health instruction, counseling and guidance to students, parents, teachers and others concerning a variety of acute, chronic health problems, first aid, diseases and other health related topics
  • Upholds professional standards, laws pertaining to health and/or nursing care and district policies
  • Assists school personnel in establishing safe and sanitary conditions in the schools
  • Collaborates with parents and other care providers in assessing, planning, implementing and evaluating care and related school health activities
  • Works closely with Pinal County and Arizona State Health Departments to ensure safety of our students and staff as well as compliance of state and federal laws and guidelines.
  • Administers first aid in accordance with established school district first aide procedures
  • Implements school policies regarding communicable disease and infection including exclusion and readmission of students
  • Identifies and reports suspected child abuse and neglect per applicable laws and school district policies
  • Identifies and reports suspected substance abuse and neglect per school district policies
  • Monitors and records immunization data per applicable laws and school district policies
  • Participates in in-service training programs
  • Attends committee meetings and conferences regarding health services and health curriculum
  • Maintains accurate and complete school health records
  • Participates in continuing education and professional development activities

 

TERMS OF EMPLOYMENT:

  • 12-month, Hourly/Non-Exempt position under FLSA.  Placement on Classified salary schedule by the Director of Human Resources dependent upon education and experience
  • Benefits – All approved benefits for Classified Employees
  • Must pass Department of Public Safety Fingerprint clearance check.
  • Must be United States citizen or have valid work visa at date of hire with the ability to maintain. All changes in visa status must be reported to Human Resources immediately. Photo identification and work visa, (if necessary), will be needed for fingerprint clearance card and I-9 documentation prior to first day of work.

 

EVALUATION:

  • Performance of this position will be evaluated in accordance with provisions of the Board’s policy on Evaluation of Classified Personnel.

 

REQUIRED DOCUMENTATION:

  • Proof of Mumps, Measles and Rubella, (MMR) vaccination.
  • Valid government issued identification or current work visa with expiration date.
  • Unofficial copies of transcripts will be accepted with applications, official transcripts will be due upon job offer*

 

* Transcripts issued in a language other than English must be accompanied by a full translation, (word for word) by an acceptable translator. Additionally, each foreign transcript must be evaluated for equivalency to United States accredited coursework by an acceptable agency such as those agencies certified by the National Association of Credential Evaluation Services, (NACES). Applications containing foreign transcripts that are not accompanied with the above required documentation will not be considered.
